If you are a healthcare entity, our records are available through the Carequality interoperability exchange, enabling seamless care coordination with multiple EMRs, including Epic, Athena, and eClinicalWorks. Please review the records exchange feature within your EMR to locate our records for a specific patient visit. If you are unable to find the visit you are looking for, please send a signed release, along with the request details, directly to our clinic’s fax number.
For diagnostic X-ray images, please initiate the request via Nuance PowerShare by searching by our facility name.
Legal firms or a record retrieval company, please submit your request for records via Chart Swap.
